Summary of Essential Functions for the Position:
Committed to Caring, The Clinical Coordinator LVN is responsible for reviewing, analyzing, uploading, and completing lab results and orders that are in the processing center folders. Provide support to the clinical field staff, patients, caregivers, and medical providers in a timely and effective manner.
Qualifications / Licensure / Certification / Knowledge / Skills / Abilities / Shift:
- Currently licensed in the state(s) in which practicing
- Licensed Vocational Nurse (LVN) preferred
- CPR certified with American Heart Association or American Red Cross
- Nursing experience with emphasis in home care preferred
- Experience with EMR System, Kinnser/Wellsky preferred
- CPR certified with American Heart Association or American
- Demonstrates knowledge of Medicare, accreditation criteria and standards
- Acceptance and ability to demonstrate support the core values and goals of Agency
- Ability to demonstrate flexibility, initiative, and work with minimal supervision
- Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal
- Ability to demonstrate critical thinking, observation, good written and oral communication skills
- Ability to demonstrate problem identification & resolution skills
- Ability to demonstrate positive customer service skills
- Must have a current driver's license, auto liability insurance and reliable transportation
- Must be on call at least once per month, or as needed
- Travel may be required
Educational Requirements:
- High School Diploma or GED. Graduate from an accredited school of nursing.
Working Conditions & Physical Requirements:
- Work environment is relatively busy and can be stressful at times. The position requires visual acuity and dexterity, sitting, standing, some pushing, pulling, and lifting up to 25 pounds. Auto related accidents possible.
Shift:
- Three (3) days a week, Friday a must
